Clinical Study on Idiopathic Dilated Cardiomyopathy in Childhood


Abstract
A clinical study for 18 patients who were admitted diagnosed as idiopathic dilated cardiomyopathy in the Pusan National University Hospital from January 1987 to June 1993 was performed.
@ES The results were as follows:
@EN 1) Among 18 patients of idiopathic dilated cardiomyopathy, 11 cases(61.1%) were yonger than 1 year of age and 17 cases(94.4%) were younger than 3 year of age. the male to female was 1 : 1.6.
2) On admission, congestive heart failure was noted 13 cases(72.2%), murmur in 9 cases(505), fever 3 cases(16.7%) and mild cyanosis 2 cases(11.1%).
3) The electrocardiography showed left ventricular hypertrophy in 66.7%, nonspecific ST-Tchange in 44.4%, left atrial enlargement in 38.9%. The other findings were right atrial enlargement, and biventricular hypertrophy.
4) On chest X-ray, cardiomegaly was noted in all 18 cases, pulmonary venous congestion 10 cases(55.6%) and pleural effusion 3 cases(16.7%). The cardiothoracic ratio on admission and during follow up after mean 34 months were 0.66 and 0.59
respectively.
5) On echocardiographic examination, left ventricular enlargement was noted in all cases left atial dilatation in 66.7%, mitral valve insufficiency in 66.7%. On admission, mean left ventricular FS, EPSS(mm) and LVIDs were 0.14, 17mm and 43.3mm
but
during follow up after mean 34 months, FS was 0.25, EPSS was 13.7mm and EF was 58.0%
6) Of alive 10 cases, 6 cases(33.3%) showed clinical improvement but 4 cases(22.4%) were not improved and 8 cases(44.4%) were expired.
